Gilead's Bictegravir/Lenacapavir Combination Reaches FDA PDUFA Date Today — First STR Studied in Patients Switching from Complex Multi-Tablet Regimens
Today, August 27, 2026, is the PDUFA action date for Gilead Sciences' (NASDAQ: GILD) New Drug Application for bictegravir 75 mg/lenacapavir 50 mg (BIC/LEN) — a once-daily single-tablet regimen for virologically suppressed HIV-positive adults.
The FDA accepted the NDA and granted Priority Review, Gilead disclosed in an April 29, 2026 news release on its investor relations site. If approved today, BIC/LEN would be the smallest single-tablet regimen (STR) for HIV treatment and the first STR studied specifically in adults who are virologically suppressed on complex multi-tablet regimens — two distinctions the company flagged in that April 29 release.
"This once-daily, single-tablet regimen brings together the high barrier to resistance of bictegravir, with lenacapavir, a first-in-class capsid inhibitor with a novel mechanism of action that has no cross-resistance with other antiretrovirals," said Dietmar Berger, M.D., Ph.D., Chief Medical Officer of Gilead Sciences.
The NDA rests on data from two Phase 3 studies. Detailed results from ARTISTRY-1 were published in The Lancet on March 28, 2026. ARTISTRY-2 showed that switching to BIC/LEN had no significant impact on weight, per Gilead's disclosed top-line results from late 2025.
Both components carry meaningful commercial pedigrees inside Gilead's existing HIV portfolio. Bictegravir is the backbone of Biktarvy, currently the most-prescribed HIV treatment regimen with over 1 million users, per the April 29 release. Lenacapavir, already approved in other formulations, is described in that same release as a first-in-class capsid inhibitor with no cross-resistance to other antiretrovirals.
Gilead reported HIV franchise sales of $20.8 billion in 2025, representing 6% year-over-year growth, per the company's disclosed financials. The April 29 release identifies the combination's label distinction — the first STR studied in adults virologically suppressed on complex multi-tablet regimens — as a patient population Biktarvy's existing label does not specifically target.
Gilead carries a market capitalization of $184.47 billion as of August 27, 2026, with 1,242 million shares outstanding for the quarter ended March 2026. The company's current cash position was not specified in accessible Q2 2026 earnings materials from its August 4 report. GILD's all-time high of $157.29 was set on February 11, 2026.
No comparable single-tablet HIV combination filings or approvals by named peers were identified in available sources as of today's date, so direct competitive benchmarking is not available here.
What's next is binary and immediate: the FDA either acts by today's PDUFA date or issues a Complete Response Letter. Gilead has not publicly disclosed any outstanding information requests or advisory committee proceedings tied to this NDA in the April 29 release. Investors watching the HIV franchise should expect a company statement today or imminently.
